Often all people need to move forward with confidence is the right information and a little bit of practice. This guide is your starting point on all things GLP-1 injections.

What are GLP-1 injections?

Most GLP-1 medications are subcutaneous injections, meaning they’re injected into the fatty layer just under the skin. They are typically delivered using single-use pens or multi-dose vials with replaceable syringes or needles. Where do you inject GLP-1 medications?

Providers typically recommend three primary injection sites for GLP-1 medications.

1. Abdomen

  • At least 2 inches away from your belly button
  • Large surface area marks rotation easier
  • Often easiest to reach and see

2. Thigh

  • Front or outer thigh
  • Avoid inner thigh and irritated skin

3. Upper arm

  • Back of the upper arm (triceps area)
  • May require assistance

These areas work well because they contain enough fatty tissue for proper medication absorption.

Step-by-step instructions for injecting a GLP-1 medication

Step 1: Wash and prepare

  • Wash hands thoroughly
  • Gather supplies (pen/needle, alcohol swab, sharps container)

Step 2: Choose and clean your injection site

  • Select a spot on your abdomen, thigh, or upper arm
  • Avoid scars, irritated skin, or moles
  • Clean with alcohol swab and let dry completely

Step 3: Prepare the pen or syringe

  • Remove cap from the pen or syringe
  • Check medication—it should be clear and colorless

Step 4: Inject the medication

  • Hold at a 90-degree angle
  • Insert needle quickly, then press and hold
  • Hold in place for several seconds before removing

Step 5: Dispose of the needle safely

  • Place needle in a sharps container or hard, puncture-proof container
  • When the container is full, drop off at your pharmacy, doctor’s office, or local collection site. Do not put it in the trash.

Why rotating injection sites matters

Using the same spot repeatedly can lead to mild redness or itching, bruising, lumps under the skin, and even changes in medication absorption. Rotating sites helps to keep skin healthy and injection sites comfortable. 

While severe reactions are rare, if you experience severe pain, rapidly spreading redness, warmth, pus, hives, facial swelling, or trouble breathing, seek urgent care immediately.

Simple tip: Change the injection site to a different part of the body with each dose. Each injection should be at least one inch away from the last one.

Injection timing

Most patients take a GLP-1 injection once a week. Exact timing depends on the medication and how your body responds to it. In general, plan to take your injection at around the same time, on the same day, each week.

✅ Swap injection site with alcohol first and let dry completely before injecting. This helps ensure the injection site is sterile.

✅ Let medication come to room temperature before using. Cold injections can sting a bit more. If your medication is refrigerated, leave it out at room temperature for a short period before injecting.

✅ Keep a site rotation log, including where you injected, site reactions (if any), and other notes you want to remember.

✅ Inspect skin regularly. Site reactions might not show up for a few days. Keeping an eye on injection sites throughout the week can help you decide on the best go-to spots for future injections.

✅ Do not reuse needles. GLP-1 needles are meant to be used just once to maintain sterility and safety; reusing needles can lead to infection or dosage inaccuracies.

Do GLP-1 injections hurt?

Most people report a mild pinch or pressure, with discomfort fading over time. Technique, needle size, and site rotation can all help you get used to the needle pricks.

Does rotating injection sites change how well the medication works?

Generally, approved sites (abdomen, thigh, arm) have similar effectiveness, although injecting at the same site every time can have an impact on absorption. Rotation is mainly about protecting your skin.

What if I miss a dose?

If you’re on a weekly injection schedule, take the missed dose as soon as you remember if it is still reasonably close to your usual day. If you’re closer to your next scheduled dose, skip the missed dose and resume your normal schedule—do not double up.
