Dosing schedule for Wegovy®

Many people have questions about dosing when starting a new GLP-1 medication like Wegovy® (semaglutide). Each GLP-1 agonist is a little different, which affects how much you will take and how quickly you will ramp up to the maintenance dose.

What are GLP-1 agonists?

GLP-1 agonists mimic a hormone the body naturally produces: glucagon-like peptide 1. This hormone helps regulate blood sugar and insulin levels, slows stomach emptying, and reduces hunger. By working on satiety pathways in the brain and gut, GLP-1 agonists help people feel full sooner and eat less while supporting better glucose control. Most have long half-lives and are taken once weekly for consistent effect.

What is Wegovy®?

Wegovy® is a once-weekly semaglutide injection that is FDA-approved for chronic weight management in adults with obesity or overweight plus a weight-related condition. It may also reduce the risk of major cardiovascular events in certain patients with excess weight and heart disease. In the large SELECT randomized trial, semaglutide 2.4 mg reduced major adverse cardiovascular events compared with placebo, with MACE rates of about 6.5 percent versus 8.0 percent and a hazard ratio near 0.80 over roughly 40 months; the Food and Drug Administration cites that evidence in the approval for this indication. For the FDA announcement, see FDA approves first treatment to reduce risk of serious heart problems specifically in adults with obesity or overweight. For the SELECT trial summary and principal results, see the American College of Cardiology coverage: Semaglutide Effects on Cardiovascular Outcomes in People With Overweight or Obesity (SELECT).

Wegovy® uses higher doses of semaglutide than some other brands in this class.

Wegovy® dosing schedule

Each person’s dosage amount and schedule is different and will be determined by their medical provider. The schedule below is just a general outline of what your dosage schedule could look like:

  • Weeks 1–4: 0.25 mg, injected weekly
  • Weeks 5–8: 0.5 mg, injected weekly
  • Weeks 9–12: 1 mg, injected weekly
  • Weeks 13–16: 1.7 mg, injected weekly
  • Week 17 and onward (maintenance): 1.7 mg or 2.4 mg, injected weekly

Why the slow ramp-up? Gradually increasing the dose helps the body adjust and reduces the chance of gastrointestinal side effects. Rapid jumps in dose tend to increase nausea, vomiting, and other GI symptoms. If side effects occur, clinicians may pause or step back to the last tolerated dose and re-escalate more slowly.

Medication is prescribed only after consultation with a licensed provider to determine appropriateness.

How to inject Wegovy®: Technique and rotation

  • Inject subcutaneously into the abdomen, outer thigh, or upper arm.
  • Clean hands and the site before injecting. Hold the needle in place for 5 to 10 seconds to ensure full delivery.
  • Rotate sites to avoid irritation and lumps. Keep injections at least two finger widths from the navel if using the abdomen.
  • If you can pinch an inch of skin, insert at a 45- to 90-degree angle.
  • Record each weekly injection to stay on schedule.

If you prefer non-injectable options, Shed also offers GLP-1 lozenges and liquid drops, which some patients find more convenient:

Missed doses and getting back on track

If you miss a weekly dose, contact your medical provider for personalized guidance. General label guidance is:

  • If your next scheduled dose is more than 4 days away, you can usually take the missed dose when you remember.
  • If your next dose is within 2 days, skip the missed dose and take your next dose on the regular day.
  • If you miss multiple weeks, do not restart on a high dose. Reach out to your provider. You may need a lower restart dose and a slower re-titration to avoid side effects.

Shed clinicians can help you decide the safest way to resume.

Common side effects and simple ways to manage them

Most side effects happen during dose increases and often improve within a few weeks.

  • GI symptoms: nausea, vomiting, diarrhea, constipation, abdominal pain, indigestion
  • Other transient effects: headache, dizziness, fatigue

Self-care tips:

  • Eat smaller, more frequent meals. Prioritize lean proteins and high-fiber foods. Avoid heavy, fried, or spicy meals during escalation.
  • Stay hydrated. Ginger tea or ginger ale can help mild nausea.
  • For constipation, increase water, fiber, and physical activity; consider a stool softener or polyethylene glycol if needed.
  • If symptoms worsen, ask your clinician about pausing, reducing the dose, or prescribing nausea medicine.

Seek urgent care for severe abdominal pain, vision changes, trouble breathing, or signs of an allergic reaction.

Wegovy®, Ozempic®, and Mounjaro®: Why dosing differs

GLP-1 medicines have different dosing ranges and titration schedules. Wegovy® and Ozempic® both contain semaglutide but are approved for different indications and use different approved dose ranges. Other agents in the class have their own dose steps and may produce different average weight losses in trials. For details and to compare options:

Wegovy® FAQs

How often do I take Wegovy®?

Wegovy® is taken weekly via subcutaneous injection, usually into the thigh or abdomen. You should take it on the same day each week. Consistent weekly dosing helps maintain stable medication levels and helps to maximize results.

What if I miss a dose?

Talk to your medical provider about what to do if you miss a dose. In general, if it's been 3 days since your scheduled dose you can take the missed dose when you remember. If it's been longer than 3 days, skip the missed dose and resume your usual schedule. After several missed doses, contact your Shed provider to discuss restarting at a lower dose.

How does dosing for Wegovy® compare to Ozempic® and Mounjaro®?

Each medication type has different dosing amounts and schedules. For an overview of dosing for Ozempic®, Mounjaro®, and Zepbound®, see the links below:

How should a clinician determine an individualized Wegovy® titration plan?

Clinicians consider baseline BMI, medical history, tolerance, and side effects. Most patients follow the standard step-up schedule, but those with more GI sensitivity may stay longer at a dose before advancing. Shed providers adjust the pace if symptoms appear, and may step back to the prior tolerated dose before re-escalating.

What are common side effects during escalation and how can they be managed?

Nausea, vomiting, diarrhea, constipation, abdominal discomfort, headache, and fatigue are common early on. Shed recommends smaller, protein-forward meals, hydration, fiber, and avoiding heavy or spicy foods. For persistent symptoms, your provider may slow the titration or prescribe anti-nausea medicine.

How does maintenance dosing differ from the initial ramp-up?

Ramp-up doses are lower and primarily about tolerability. Once at maintenance, the dose stays consistent weekly to maintain appetite control and weight effects. If side effects occur at a higher dose, your provider may recommend a lower maintenance dose rather than forcing the maximum dose allowed.

When should a provider adjust maintenance dose for a plateau?

If weight loss plateaus for 8 to 12 weeks despite good adherence, nutrition, and activity, a provider may consider a dose increase if tolerated, or reinforce lifestyle strategies like protein targets and resistance training. Shed clinicians also review timing of injections, side effects, and potential medical contributors.

Why do GLP-1 dosing schedules vary between brands?

Different molecules and formulations have different potency and half-lives, so approved dose steps and maintenance targets vary. Wegovy® and Ozempic® contain semaglutide at different dose ranges, while other agents have distinct schedules. Your Shed provider will match the regimen to your goals and health profile. What injection technique and site rotation guidance should patients follow?

Inject into the abdomen, outer thigh, or upper arm, and rotate sites regularly. Clean the area, pinch an inch of skin when possible, inject at 90 degrees, and hold for several seconds. Shed provides step-by-step training and support.

What are the pharmacologic reasons GLP-1 agonists reduce appetite?

GLP-1 agonists activate receptors in the brain and gut that enhance satiety signals and slow gastric emptying. This makes people feel full sooner and for longer, which reduces calorie intake. Weekly dosing maintains steady receptor activation.

How can a telemedicine clinic implement safe prescribing and monitoring?

Telehealth programs should verify state licensure, collect medical history, assess labs when indicated, provide clear education on side effects and injection technique, document informed consent, and schedule close follow-up during dose escalation. Shed integrates these steps into every program.

What patient selection criteria and monitoring steps should providers use?

Candidates typically have obesity or overweight with a weight-related condition. Providers review medical history, assess medications, and monitor for side effects. Shed clinicians track progress, side effects, and dose tolerance, and adjust the plan as needed.
